How do I get started?

To get started with credentialing please complete the provider intake form and schedule your credentialing consultation with us.

Why do you ask for my personal information?

Your personal information is required for any state or federal insurance application. Your information is verified to ensure that you are a U.S. citizen and that you don’t have any disqualifying events for participation in provider enrollment.

How long will it take for my application to be submitted?

Client application time varies, once your documents are submitted and verified the application process begins within 24-48 hours. If there are any problems with your documents you will be contacted by a staff member.

What documents are required to complete your provider enrollment application?

6 documents are required for a smooth credentialing process and are as follows:
*Business License or Other Business Formation Document
*CP 575 Letter (EIN Letter from the IRS)
*W-9 Form (completed and signed)
*Proof of Business Bank Account (Voided Check or Bank Letter)
*CLIA Certificate/ Number
*NPI Number Kindly send them separately, in Portable Document Format (PDF)
